What version of acrobat do you have? The document was created with version 6. Try upgrading if not running the latest.

Thank you I was on Acrobat 4, I've upgraded and all is now OK.

Some time since thread - I cannot find archive material either. Are the 355 codes still available anywhere on PDF or wherever? I have a euro version 1997. Thanks in advance.
